Dr. Lucas Adam presented findings from the PoCoVIT trial, which evaluated methylprednisolone as a treatment for cognitive impairment in patients with post-COVID syndrome (PCS). The investigators aimed to determine whether immunosuppression could improve memory and other cognitive symptoms in a well-defined subgroup of PCS patients. Analysis of the available data showed that both the methylprednisolone and placebo groups experienced some improvement in symptoms over the course of the study. However, there were no statistically significant differences between the groups for either the primary endpoint or any of the assessed secondary outcomes, indicating that methylprednisolone did not provide measurable clinical benefit under the conditions studied. The study was terminated early in March 2025 following the occurrence of serious adverse events in the treatment arm. These included thromboembolic complications, which were observed exclusively among participants receiving methylprednisolone. As a result, recruitment was halted before the planned sample size was reached. Despite the negative findings, the trial was presented as an important contribution to the field because of its rigorous design and strong mechanistic rationale. Rather than testing a broadly defined Long COVID population, the study specifically focused on patients with cognitive impairment linked to plausible inflammatory mechanisms. Ongoing analyses of biological markers and predefined patient subgroups may still provide valuable insights into disease mechanisms and help identify individuals who could respond to more targeted therapies in future studies.
